Job Description:
• Support and coordinate the mental and physical health care needs of patients on an assigned caseload
• Screen and assess patients for common mental health and substance abuse disorders and facilitate patient engagement and follow-up care
• Provide brief behavioral interventions using evidence-based techniques
• Track patient follow up and clinical outcomes using a registry
• Participate in regularly scheduled caseload consultation with the psychiatric consultant
• Support psychotropic medication management as prescribed by medical providers
• Facilitate coordination of care to available resources as needed and provide social work case management
Requirements:
• Education: Licensed Clinical Social worker, Licensed Professional counselor or Licensed in a related field of study
• Completion of degree and all post-degree supervision for licensure and at least 2 years of post-degree experience
• Experience of successfully delivering evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders
• Ability to effectively engage patients in a relatively short-term therapeutic relationship, via video and/or audio
• Evidence of high degree of organizational, collaborative and communication skills
• Licenses: Licensed in one or more states. Willingness, education and experience sufficient to obtain multiple state licenses.
