Care Coordinator

Our diverse team of clinicians and support staff is passionate about making a positive impact in the lives of our clients. As we expand our reach, we are looking for a compassionate, highly organized Care Coordinator to serve as a key point of contact for clients and support them through the coordination of their care. Role Description The Care Coordinator plays a vital role in ensuring clients have a seamless and supported experience as they engage in services. This position requires excellent communication skills, attention to detail, and the ability to balance administrative responsibilities with client-centered coordination.
