About the position
This position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives. The role involves obtaining in-depth social histories, developing and assuring community and/or agency resources to meet the consumer's needs, serving as a Qualified Intellectual Disabilities Professional (QIDP), and providing monthly program monitoring. The position requires compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines. This role is located in Oklahoma City and Chickasha, Oklahoma.
Responsibilities
• Provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives.
• Obtains in-depth social histories.
• Develops and assures community and/or agency resources to meet the consumer's needs.
• Serves as QIDP.
• Provides monthly monitoring of the program.
• Meets compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines.
Requirements
• Bachelor's degree in a human services field and one (1) year of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field.
• OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and one year of experience working directly with individuals with intellectual and/or developmental disabilities.
• Bachelor's degree in a human services field and two (2) years of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field.
• OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and two years of experience working directly with individuals with intellectual and/or developmental disabilities.
• Must possess a valid driver's license.
• Must maintain required car insurance.
