We are looking for compassionate, motivated Community Health Workers (CHWs) to join our growing telehealth team. In this role, you will serve as a trusted bridge between Medicare patients and the health and social services they need. You will work directly with patients over the phone to understand their personal challenges, connect them to resources, and provide ongoing support — all from the comfort of a remote setting.
This is a meaningful, patient-facing role ideal for candidates who are already Medicare-enrolled and ready to hit the ground running from day one.

WHAT YOU'LL DO
━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━
• Conduct regular phone-based check-ins with assigned Medicare patients
• Assess patients' social, financial, and health-related needs
• Help patients navigate the health care system and understand their benefits
• Assist with scheduling and coordinating medical appointments
• Connect patients to community resources including food assistance, transportation, housing support, and utility programs
• Provide guidance on managing prescriptions and medication schedules
• Help patients understand and organize their medical bills and paperwork
• Support patients in arranging home health care and medical equipment
• Document all patient interactions accurately including time spent, activities performed, and outcomes
• Communicate regularly with the supervising clinical team regarding patient needs and progress
• Maintain strict patient confidentiality in accordance with HIPAA requirements

✅ Currently enrolled or opted-in with Medicare

ADDITIONAL QUALIFICATIONS
━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━
• High school diploma or equivalent required; associate or bachelor's degree preferred
• Experience working with elderly or Medicare populations strongly preferred
• Strong communication and active listening skills
• Empathetic, patient, and professional demeanor
• Comfortable working independently in a remote/telehealth environment
• Reliable phone and internet connection required
• Bilingual (English/Spanish) a plus
• Familiarity with community resources in Illinois a plus
• Must be willing to complete paid competency training upon hire
