As a remote medical case manager and advocate, you will work with a registered nurse case manager to assess, plan, and advocate for the needs of our members. You will be responsible for researching available options and resources for members with financial, transportation, safety, and housing needs, empowering them to make positive lifestyle and medical choices, and working as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health. **Responsibilities** * Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs * Research available options and resources for members with financial, transportation, safety, and housing needs * Empower members to make positive lifestyle and medical choices * Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health * Conduct telephonic or electronic exchanges with members to assess their needs and provide support * Develop and implement individualized plans to meet the unique needs of each member * Collaborate with other healthcare professionals to ensure comprehensive care and support for members * Maintain accurate and up-to-date records of member interactions and progress **Qualifications** * Must have an active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence * 2+ years of experience in medical social work or case management * Masters degree in social work or a related field * Strong communication and interpersonal skills * Ability to work independently and as part of a team * Strong problem-solving and critical thinking skills * Ability to maintain confidentiality and handle sensitive information * Strong computer skills and proficiency in electronic medical records systems **Benefits** * Competitive salary range: $46,597.20 - $101,228.40 * Comprehensive benefits package, including medical, dental, and vision insurance * 401(k) retirement savings plan and employee stock purchase plan * Paid time off (PTO) and paid holidays * Free development courses and education assistance * CVS store discount and discount programs with participating partners * Fully-paid term life insurance plan and short-term and long-term disability benefits * Well-being programs and employee assistance program **Work Environment** * Remote work from home role with the ability to work anywhere in the U.S. * Monday-Friday, 8am-4:30pm Pacific Standard Time * No weekends or holidays required **How to Apply** If you are a compassionate and skilled social worker looking for a challenging and rewarding role that allows you to make a meaningful difference in people's lives, we encourage you to apply for this exciting opportunity.
