**Job Summary**
We're seeking an experienced remote social worker to join our team as a Medical Case Manager and Community Resource Coordinator. This role will partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. As a remote social worker, you'll work with members to research available options and resources for financial, transportation, safety, and housing needs. This position requires an LSW/MSW with an unrestricted active license and 2+ years of experience in medical social work or case management.
**Responsibilities**
As a remote social worker at arenaflex, your key responsibilities will include:
* Collaborating with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs
* Researching available options and resources for members with financial, transportation, safety, and housing needs
* Empowering members to make positive lifestyle and medical choices
* Working as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
* Conducting telephonic or electronic exchanges with members to provide support and guidance
* Maintaining accurate and up-to-date records of member interactions and outcomes
**Requirements**
To be successful in this role, you'll need:
* An active and unrestricted LSW - Licensed Social Worker, LMSW - Licensed Masters Social Worker, LCSW - Licensed Clinical Social Worker, or LISW - Licensed Independent Social Worker license in the state of residence
* 2+ years of experience in medical social work or case management
* A Master's Degree in Social Work
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
* Strong problem-solving and critical thinking skills
* Proficiency in electronic medical records and other software applications
