**Job Summary:**

**Responsibilities:**
- Collaborate with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs.
- Research available options and resources for members with financial, transportation, safety, and housing needs.
- Empower members to make positive lifestyle and medical choices through education and support.
- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
- Utilize telephonic and electronic exchange to communicate with members and other stakeholders.
- Develop and maintain relationships with community resources and organizations to ensure access to necessary services and support.
**Essential Qualifications:**
- Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence.
- 2+ years of experience in medical social work or case management.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently and as part of a team, with a focus on collaboration and problem-solving.
- Strong analytical and problem-solving skills, with the ability to think critically and creatively.
- Proficiency in electronic medical records and other relevant software systems.
**Preferred Qualifications:**
- Experience working in a healthcare setting, with a focus on medical case management and community resource coordination.
- Knowledge of community resources and organizations, with the ability to develop and maintain relationships with these stakeholders.
