Hera provides a dedicated care manager in the community, covered by Medicare, to be the trusted guide to age at home. We help the family just be family again.
We are hiring for 2 roles for social workers that are both full-time operating autonomously in a fully remote environment.
• Care Manager Lead (W2): supervisor to other care managers and managing a pod as well as a smaller caseload of higher-acuity families
• Care Manager (1099): managing your own clients and serving as a dedicated partner to older adults and their families
These are both client-facing roles your time is spent directly helping families navigate complex systems, coordinate care, and advocate for what their loved ones need.
Care Manager Lead:
• Pay: $40-50 / hour
• Benefits: Premium Health Insurance
Care Manager (1099):
• Pay: $40-60 / hour
You’d Be a Great Fit If You:
• Love creating connection with families and older adults
• Find yourself to be a creative problem-solver and want a job that looks different every day
• Are tech-savvy and excited to work in a fast-paced, early-stage environment
• Hold an LMSW or LCSW (clinical license is a plus but not required)
• Have experience in care management, hospice, palliative care, or geriatrics
