Role Description
We are seeking an experienced and patient-centered Patient Outreach Medical Assistant to support our Primary Care teams by connecting with FMCOH patients recently discharged from the hospital. This role plays a critical part in ensuring safe transitions of care, preventing readmissions, and strengthening continuity with our Primary Care physicians.
• Review discharge information, reconcile medications, and schedule timely follow-up appointments across 16 Primary Care offices.
• This position is ideal for an MA who enjoys patient interaction, is confident reviewing medications, and thrives in a structured outreach/call-center–style workflow.
Qualifications
• High school graduate or equivalent.
• Successful completion of Medical Assistant training from an approved technical school (or at least one (1) year of verifiable and applicable work experience as a Certified Medical Assistant in a patient care setting) required.
Requirements
• MUST HAVE 2-3 YEARS EXPERIENCE AS A CERTIFIED MEDICAL ASSISTANT.
• Maintains current BLS Healthcare Provider Certification.
• Maintains current certification or registration by one of the following agencies:
• Certified Clinical Medical Assistant (CCMA) by National Health Career Association (NHA)
• Certified Medical Assistant (CMA) by the American Association of Medical Assistants (AAMA)
• Clinical Medical Assistant Certification (CMAC) by the American Medical Certification Association (AMCA)
• National Certified Medical Assistant (NCMA) by National Center for Competency Testing (NCCT)
• Nationally Registered Certified Medical Assistant (NRCMA) or Nationally Registered Certified Advanced Medical Assistant (NRCAMA) by the National Association for Health Professionals (NAHP)
• Registered Medical Assistant (RMA) by American Medical Technologists (AMT), the American Registry of Medical Assistants (ARMA), or by American Allied Health (AAH)
• Applicants with an accepted Medical Assistant certification or registration from an agency not listed will be considered on a case-by-case basis.
