Role Description
The Wellness and Recovery Specialist - Certified Peer Specialist is responsible for care coordination and/or care management activities focused on the wellness and recovery of members.
• Responsible for coordinating and managing care activities with a focus on the wellness and recovery of members.
• Identify opportunities to engage members and their families in forming a supportive recovery network.
• Develop and implement onsite psychiatric discharge planning education at Recovery and Resiliency sites.
• Collaborate with Stabilization Teams, advocating for members in discharge planning education, resolving barriers, and facilitating service transitions.
• Serve as a resource for staff with decision-making and problem-solving.
• Initiate and maintain contact with assigned individuals and providers to assess members' responses to services.
Qualifications
• Requires an AA/AS and a minimum of 2 years of experience in health services or behavioral health, or a peer support services role, or a combination of education and experience providing an equivalent background.
• Valid, active, certified peer specialist.
Requirements
• Knowledge of care coordination and case management concepts is highly preferred.
• A BA/BS is preferred.
