Overview
We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT) to join our team. The ideal candidate will possess a background in social work and therapy, with a focus on providing high-quality care to individuals.
This is a fully remote role.
Both experienced licensees or new licensees (LCSW/LMFT) are welcome to apply.
Responsibilities
- Conduct individual therapy sessions using evidence-based therapeutic approaches such as cognitive-behavioral therapy (CBT), motivational interviewing etc.
- Makes mental health diagnoses as part of a treatment plan as needed.
- Conducts psychosocial assessment of patients upon admission, annually, and/or as clinically indicated to evaluate social, emotional, and physical needs by applying social work theories and techniques. Identifies and addresses barriers to care.
- Manages independently assigned caseload of varying clinical acuity (mild to moderate).
- Provide comprehensive assessments and diagnoses for clients across the lifespan.
- Develop and implement treatment plans based on client needs and goals.
- Maintain accurate and up-to-date medical records.
- Provide crisis intervention services as needed.
- Experience working in Federally Qualified Health Centers (FQHC), Community Health Centers, Primary Care Outpatient Clinics, Tribal Indian Health Centers; Medi-Cal, Medi-care patients preferred.
- Prepares social case histories with particular emphasis upon psycho-social factors affecting the patient.
- Cooperates with staff and other community agencies in carrying out treatment plans.
- Performs other related duties, as required.
Requirements:
- Master's degree in Social Work (MSW) from an accredited program.
- Current California state licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT); or those who are looking to get licensed within the next 45 days.
- Experience working in a clinical setting, preferably with a focus on Medicare and Medi-Cal populations.
- Excellent interpersonal skills and the ability to establish rapport with patients from diverse backgrounds.
- Strong assessment and diagnostic skills.
- Knowledge of behavioral health and the skills to assess mental health functioning, depression, anxiety, or other psychiatric conditions impacting hospitalization or transitions of care.
- Proficient in using a computer to accurately enter and extract data, send and receive email, calendar appointments, and use task lists as will be using a variety of computer software programs.
- Bilingual (Spanish) is preferred, but not required

Pay: $104,000.00 - $140,000.00 per year
