Employment Status: Full time Shift: Day (United States of America) Facility: 1615 Franklin Rd - Roanoke Requisition Number: R159871 LPN- Home Health (Open) How You’ll Help Transform Healthcare: The LPN Home Health provides nursing care to patients of all ages in the home under the direction and supervision of the RN case manager in accordance with the plan of care and physician orders. Care must be individualized, goal-directed, and documented accurately to support coordination and discharge planning. LPN Home Health. Roanoke region. company equipment. Mileage reimbursement. Provides skilled nursing care and treatments to patients of all ages as assigned by the RN. Assists the RN case manager in coordinating all aspects of patient care. Completes and submits accurate clinical documentation within agency timelines. Complies with all hospital, departmental, and regulatory policies and procedures. Participates in continuing education, staff meetings, and required competencies. Provides customer service with respect, kindness, and empathy to patients, families, and staff. Provides care in patient homes, which may involve limited space, temperature extremes, environmental hazards, or equipment transport. Drives between patient homes daily, often in variable or adverse weather conditions. Follows infection prevention practices and safely handles blood and body fluids. Works independently with minimal direct supervision and collaborates effectively with the care team. What We Require: Education: Graduate of an accredited practical/vocational nursing program. Experience: At least one year of medical-surgical nursing experience Licensure/Certification: Current Licensed Practical Nurse (LPN) licensure in the Commonwealth of Virginia required. Must hold a valid Virginia driver’s license. Life Support: AHA BLS-HCP required when providing direct patient care. Other Minimum Qualifications: Must demonstrate the ability to perform thorough assessments and technical nursing care under RN supervision. Must be able to work independently in home environments, manage documentation requirements, communicate effectively, and interact professionally with patients, families, and other staff.
