Provide medication therapy management services to Molina Healthcare members.
Ensure safe and effective use of medications by reviewing and monitoring medication regimens.
Conduct comprehensive medication reviews and make recommendations for optimizing therapy.
Collaborate with healthcare providers to develop and implement medication management plans.
Communicate with members to educate them on medication usage, potential side effects, and adherence strategies.
Maintain accurate and up-to-date documentation of all interactions and interventions.
Adhere to all state and federal regulations and guidelines related to medication therapy management.
Participate in ongoing training and development opportunities to stay current on best practices in medication therapy management.
Collaborate with other healthcare team members to promote integrated care for members.
Maintain confidentiality and adhere to HIPAA regulations at all times.
Act as a liaison between members, healthcare providers, and the pharmacy team.
Monitor and report on medication utilization and outcomes to identify opportunities for improvement.
Participate in quality improvement initiatives and assist with implementing changes to improve patient care.
Maintain an organized and efficient workflow to ensure timely completion of tasks.
Provide excellent customer service to members and internal stakeholders.
Perform other duties as assigned by the supervisor.
