Company Description: HealthHelper is a technology-enabled services company that partners with Primary Care Practices to help manage patient panels and ensure patients receive recommended follow-up and preventive care (e.g., diabetes care, annual visits, breast cancer screening, colon cancer screening). We do so by off-loading the already overworked practice staff and reaching out to patients in a convenient, efficient, and easy-to-use manner to help them schedule and navigate their care.

Position: Primary Care Population Health Coordinator
Location: Work from Home
Work in Collaboration With: Director of Operations, Chief Medical Officer, and Chief Product Officer internally. Will also coordinate virtually with office staff at customer sites — primarily Practice Administrators, Medical Assistants, Nurse Care Managers, and Front Office Staff.
Position Summary: The Population Health Coordinator plays a central role in ensuring patients are scheduled for important primary care visits, including chronic care follow-ups, annual wellness visits, and preventive screenings. The primary focus of the position is on patient scheduling and communication, with additional support for population health efforts. This role is ideal for someone with primary care scheduling experience who enjoys helping patients access timely, high-quality care. Population health experience is preferred.
This position offers growth opportunities, including the chance to advance into a Team Lead role. Coordinators will also contribute feedback to help refine outreach and scheduling processes.
Responsibilities:
• Monitor outbound outreach campaigns
• Respond to inbound responses, requests, and questions
• Make outbound phone calls as part of the outreach campaigns
• Work with various EHRs and population health technology systems to pull reports on patients and care gaps. athenaOne experience strongly recommended.
• Assist with basic documentation in our databases as well as in customer electronic health records
• Collaborate with leadership team to iterate and innovate
• Manage patient outreach, care gap, and quality metric lists
• This is a remote, work-from-home position
• We are primarily looking for candidates in the New England and Mid-Atlantic areas
Ideal Work Experience:
• Primary care scheduling experience (REQUIRED) – e.g., experience working as a scheduler, medical assistant, or front desk staff in a medical office
• Population health management (PREFERRED) – e.g. experience helping patients get preventive and diagnostic care, for example as a Population Health Manager or Patient Outreach Coordinator for a population health program
• Technology – e.g. experience and comfort working with various healthcare electronic health records and communication technologies
