Job Description:
• Support the execution and the ongoing refinement of Population Health programs including workflow optimization in Epic.
• Work closely with functional leaders (Product, Finance, Data Science) in the central office to assist with Transitions of Care program strategy and design.
• Work with regional leaders and Care Teams to ensure effective implementation of key Oak Street clinical and business priorities.
• Lead communication and content development related to key population health metrics, including quality measures, utilization measures, and financial measures.
• Manage admission notification strategy in partnership with Product and Data engineering, including vendor management.
Requirements:
• At least 2 years of experience in healthcare, consulting, or business operations.
• Experience in care delivery / care management settings or at a value-based care organization.
• Bachelor's Degree
