Role Overview
People Concern Counseling, LLC is seeking a dedicated Senior Clinical Social Worker to join our compassionate team in delivering high-quality care within a remote hospice setting. Key Responsibilities
• Provide comprehensive psychosocial assessments and develop individualized care plans for patients.
• Deliver therapeutic interventions and support to patients and their families, focusing on emotional and social needs.
• Collaborate with an interdisciplinary team to ensure holistic care and seamless service delivery.
• Facilitate support groups and educational workshops for families and caregivers.
• Maintain accurate and timely documentation in compliance with organizational and regulatory standards.
• Stay informed about community resources and facilitate referrals as needed.
Required Qualifications
• Master's degree in Social Work (MSW) from an accredited institution.
• Current and valid state licensure as a Clinical Social Worker.
• Minimum of 3 years of clinical experience, preferably in hospice or palliative care.
• Strong interpersonal skills and the ability to work effectively with diverse populations.
• Excellent communication skills, both written and verbal.
