Liquid Personnel is recruiting an Adult Social Worker to join its client’s Integrated Neighbourhood, based in Bury.
What will your responsibilities be?
In this role, you will:
Provide statutory social work support, delivering professional social work interventions and fulfilling legal duties under adult social care legislation.
Promote independence and recovery by completing strengths-based assessments and supporting individuals to achieve the best possible outcomes.
Coordinate safe hospital discharges, working closely with health partners to ensure timely, appropriate discharge planning and effective community support.
