Role will be based out of Concord but will cover Alameda, Concord, Roseville, Sacramento, San Francisco, San Mateo, Santa Rosa, Saratoga, and Yuba. Responsible for leading and overseeing the home health and hospice hospital liaison team to support seamless transitions from inpatient care to post-acute care services. Ensures that patient referrals, admissions, and discharges are coordinated efficiently and compassionately, while maintaining compliance with regulatory standards and organizational policies. Serves as a strategic partner to hospital leadership, physicians, and interdisciplinary teams, promoting Sutter Health as the provider of choice for post-acute care services.
